Chase Allen Kizima was stillborn on September 17, 2010 in UPMC Horizon Hospital in Farrell.
He is survived by; his parents, Stanley Kizima and Aimee Kuchmak of Sharon, PA; 3 brothers, Ryan Kuchmak, Dylan Tatta, and Kyle Kizima; Maternal Grandmother, Emmy Jablon; Paternal Grandparents, Ronald and Darlene Kizima; Maternal Great Grandmother, Donna Jean Michael; 2 Aunts, Donna Miller and Shannon Jewell; 4 Uncles, Ronald Kizima Jr., Gary E. Weir, Jeff Jewell, and Dan Miller.
Arrangements are entrusted to the J. Bradley McGonigle Funeral Home and Crematory, Inc. 1090 E. State St. Sharon, PA 16146.
